Instructions: discussions should be approx. 750 words (you will not be downgraded if you go over). please include word count with your post. all in text citations, quotations and paraphrasing should be done in apa format. references should not be included in word count. this weeks assignment requires you to critically analyze a clinical scenario. you are asked to develop a diagnosis for a patient. please note, you will be graded on the process and your critical analysis of the information provided. you must support your diagnosis with data and cited information. it would be helpful to explain other potential pathologies and how/why you excluded them. thus, the diagnosis itself is not the determining factor for grading, a 23-year-old man presented with a 4-week history of coughing, breathlessness and malaise. he had lost 4kg in weight, but had no history of night sweats or haemoptysis. he had returned from holiday in pakistan 2 months earlier. on examination, he was mildly pyrexial (37.8°c) but had no evidence of anaemia or clubbing. crepitations were audible over the lung apices; there were no other physical signs. his haemoglobin and white cell count were normal but the crp was 231 mg/l. the chest x-ray (links to an external site.) showed bilateral upper- and middle-lobe shadowing but no hilar enlargement. sputum was found to contain acid-fast bacilli and mycobacterium was subsequently cultured. a mantoux test (links to an external site.) was strongly positive. what is your diagnosis and why?
