Setting and Access

Over the course of twelve weeks, the goal of the program would be to decrease the number of people dying of congestive heart failure (CHF) or being hospitalized due to heart failure by about 10% using a smartphone-based program. Heart failure survivors, would be the target demographic. The team consists of project manager, other medical practitioners, and other personnel who will receive training. Financing will come from non-governmental agencies, and the location of the headquarters would be a medical center of choice. The smartphone-based program would have a hospital setting that would have specialized outpatient services for the heart failure patients. That is, the program would have special features that would allow patients to communicate effectively with the care nurses and other team members regarding their recovery processes or any other emerging issues.

Timeline for The Project

The project will take an approximated duration of about 12 weeks. The duration is enough considering that it will be the first phase or a pilot study that would not include many healthcare facets. The program will focus on using elements are being applied in the institution to have a better compatibility with the existing treatment options and management strategies for CHF patients. The implementation phase would a selection of the monitoring components that would be used to throughout the program.

The Delivery Process and Training Required

Initial Meeting

At this stage, a face-to-face meeting will be held. Here, we'll figure out what works and what doesn't in terms of the listed processes, as well as what we really have to know and what we might need as we read more about congestive heart failure and what exactly is expected to have a solution.

Assessment of The Program

We can use an input form or a questionnaire at this point to give us an estimate of the strength proposals and to establish a timeline. In essence, this would allow us to gather important details that will be used to establish the most appropriate solution. The input forms would be designed using the most basic language that would be considerate of sensitive issues that would affect the patients’ psychological health.

Follow-Up Meetings

These follow-up sessions will allow us to devise a solution and a strategy. At this stage, we discuss solutions, alternatives, and add-ons, as well as make adjustments to the current response strategy. For example, the participant patients would be asked about how they are fairing with program, whether there were difficulties while accessing the platform, or enquire about how easy it has been for the CHF patients to use the smartphone-based program. Besides, suggestions would be welcomed from the patients concerning how they would like the program to be.
