Instructions: The purpose of this assignment is to begin the preparation for your capstone project, which will be implemented in nur 464. *please be aware that your capstone project will actually be implemented and requires a synchronous or live component of interaction with individuals. simply hanging a poster and collecting responses is not appropriate. your faculty can provide further guidance, but as you start to prepare and propose a project keep in mind that there needs to be interaction with people.* you must present your project to people and not just send a survey to people after viewing a power point. for this assignment, please use the provided template/outline. over the two courses (nur 445 and 464), you will propose and plan a project by identifying a problem or need for change to address a quality or patient safety/community issue as defined in the next paragraph. propose a solution that is supported by evidence (evidenced based) identify the process of change, including the costs and benefits to the stakeholders determine the criteria for the evaluation of the project outcomes. this is not a study. please keep your proposal focused on a single issue, not complex or complicated. this assignment is to learn how to propose and plan, not solve all the compelling issues in nursing today. if you stay focused on one issue or problem, the assignment will be much easier to complete. you cannot do patient chart reviews to obtain data (this requires irb approval) address a quality care or patient safety issue: identify one issue that exists within an inpatient, outpatient, or community setting. review the issue utilizing management functions: organizing, planning, directing, controlling, and budgeting, and integrate each of these functions into the proposal. consider the impact of change to the organization's mission, staffing, scheduling, staff development, staff and patient satisfaction, and budgetary performance, if applicable. identify all stakeholders, such as finance, human resources, nursing, medical staff, pharmacy, patients/families or general public, and plan for coordination during implementation. determine how cost-benefit and quality will be affected, measured, and evaluated. please note: very important.....this is to be a student project, not a study. the faculty will not approve a capstone project that is a study. a decision tree is attached to these instructions for you to use to determine if you are designing a project that is more like a study and would require irb approval. you may need to use the "enable editing" function to see the decision tree clearly. this is an individual student project, not a group project. you can work together with other students in the course to develop a mini health fair where each student individually presents their project to the community, but each student must individually present a project. this assignment is the first step used to design the implementation of the nursing practice project as required in your capstone course, nur 464. the project needs to be able to be implemented and evaluated over a 4-week timeline, so please consider this time frame when designing your project. for the interprofessional question on the outline- you must identify a discipline other than nursing (this includes cnas and nps). examples include respiratory therapy, physical therapy, nutrition services, information technologist, pre-hospital services, infection control specialist, etc...it does not need to be a health discipline. think big! Identification of the Problem

The issue or problem is the patient’s lack of education regarding diabetes self-management. The issue is within the scope of my practice to change because I am the first point of contact with patients. My capstone project does not require IRB approval because it focuses on empowering diabetic patients to take control of their condition

Picot Question

In diabetic patients, does a diabetes self-management program compared to usual care improve glycemic control within four weeks?  I will use the following databases to answer my PICOT question: Google Scholar, PubMed, CINAHL, and the Cochrane Library.

My Overall Objective

My overall objective is to empower diabetic patients to manage their condition by educating them about the importance of healthy eating, physical activity, monitoring their blood sugar levels and adhering to their treatment regimen.

Who Needs to Approve My Project

My Unit director and the hospital administration need to approve my project because it is a health improvement initiative for diabetic patients and those at risk of diabetes.

The estimated Cost of Implementing My Idea

The approximated cost is $280/ per patient/ annually. The cost will include travel expenses and the printing of flyers to increase individuals’ awareness of diabetes self-management techniques.  Healthcare providers will use their time screening those at risk of diabetes and educating them on the importance of healthy eating and physical activity to reduce their risk of being diabetic.

The General Time Frame for My Project

The project will take four weeks. The activities will include designing the program’s content and printing flyers and brochures to create awareness among the target patients about the importance of healthy eating, physical activity, blood glucose monitoring, and medication adherence. Healthcare providers will also be educated on the need to screen those at risk of diabetes and educate them on the importance of healthy eating and physical activity to reduce their risk of getting the disease.

Evaluating the Effectiveness of the Project and Data

The program’s effectiveness will be evaluated using survey questions and interviews.  The patients should verbalize the self-management strategies to control diabetes. Additionally, the number of patients visiting the facility because of abnormal blood sugar levels should considerably decline.  I will ask my colleague to help me obtain data about diabetic patients and whether their admission rate has increased or declined. The data I need will be readily available in the facility’s database.

One Discipline that will be involved other than Nursing

 I will need an exercise trainer to implement my program effectively. The exercise trainer will design fitness programs that reflect individual patients’ preferences.
