What necessary information would need to be obtained about the patient through health assessments and diagnostic tests?

Consider how you would respond as an advanced practice nurse. Review evidence-based practice guidelines and ethical considerations applicable to the scenarios you selected.

Write a detailed one-page narrative (not a formal paper) explaining the health assessment information required for a diagnosis of your selected patient (include the scenario number). Explain how you would respond to the scenario as an advanced practice nurse using evidence-based practice guidelines and applying ethical considerations. Justify your response using at least three different references from current evidence-based literature.

Case Scenario 1

The case represents a 27-year-old male that is suffering from crohn disease and has now been admitted to the emergency room with an extreme form of flare up of his current condition.  The patient the explains that he has not been in a position to afford any medication in the past few months, thus he is concerned about the cost that he will incur in the process.

Narrative

Based on the patient condition and situation, it is evident that he is stressed up due to the crohn disease.  First, the approach that would be appropriate will be to inform the patient on the intention to contact the care coordinator in the hospital to discuss various ways of assisting the patient (Nielsen & Ainsworth, 2018). This is important as it helps in reducing the level of stress that the patient might be going through.  It is also important to discuss the patient’s problem and weigh if it is actually caused by Crohn or if this could be another Gastrointestinal issue (Nunes & Everts, 2019). It is important to look at the patient’s symptoms, I would also ask the patient when these symptoms began. It is also important to find out whether the patient is in other medication for crohn’s disease since he is not in the position to afford them, it is also important to find out the current medication.  Also, I would look for other medical challenges that he could be facing, and the family history with Crohn. I would look at his lifestyle habits like smoking and the food he takes as they are often triggers for the condition (Pasha  & Leighton, 2009).  Lastly I would order for a CBC test with differential, hemoccult of stool, stool culture and also refer the patient to a gastrointestinal doctor for a colonoscopy.
