What will professionals need to be involved in managing Peter's continuing care, both short and long-term?

Peter’s condition requires an interdisciplinary approach to help manage his current condition, with short-term and long-term goals of improving his health outcomes. Each of the professionals has a significant role in addressing the problems which Peter is currently experiencing. They include; a neurologist who will assess the extent of damage to Peter's brain by carrying out various imaging studies such as CT and MRI and assessing the extent of the motor and speech disabilities (Levitt & Addison, 2018). A physiotherapist is also a significant member of the team and is responsible for promoting the muscles' function and improving muscle strength, which may promote his ability to walk. A speech therapist will help improve his ability to communicate by identifying his needs and analyzing his options. A psychiatrist will assess his mental well-being, make a diagnosis and prescribe medication that will help in managing depression which the patient may be undergoing (Levitt & Addison, 2018). Lastly, social workers and community health workers are also essential in offering him social support and counseling to enhance his adaptability to his current condition.

An interdisciplinary approach will address all the problems which peter is experiencing. It will provide the best quality of life possible for Peter since it will improve his skills, provide him with various opportunities in the community and even build his confidence (Ciemins et al., 2016). Besides, it encourages family support for Peter, which is paramount since the family will provide him with the resources he may need. He will not feel left out because of his condition. The ultimate impact is and enhances mental and emotional development for the patient.

An interdisciplinary approach allows the integration of different health services, which will improve Peter's health outcomes. This is through approaching the patient holistically, therefore, addressing all his needs and problems. The medical care providers involve the patient in all decisions concerning his health, promoting autonomy and respect for the patient (Ciemins et al., 2016). They work collaboratively with a common goal of treating Peter, and this will improve his level of satisfaction and lead to a high quality of care.

Identify any potential barriers to effective teamwork in this case? How could these be overcome?

Different factors can hinder effective interdisciplinary collaboration. These include; lack of effective communication among the team members. However, this can be overcome by creating a platform where all team members can freely communicate and express themselves (Dirks, 2019). Secondly, leadership ambiguity and disunity among team members will hinder effective teamwork. Addressing this involves learning about the importance of each profession in caring for the patient and by learning to respect everyone's knowledge and skills. Lastly, the lack of an appropriate work framework will also negatively impact the effectiveness of the team.  This can be overcome by conducting meetings whereby a working framework is formed (Dirks, 2019). This will save on time and will also ensure that the patient’s appointments do not overlap.

What will professionals need to be involved in managing Antoinette's continuing care, both short and long-term?

The interdisciplinary team will consist of various professionals, each with a specific role crucial in enhancing the patient’s health outcomes. Firstly, a primary health care provider, for instance, a family doctor, will provide regular checkups for Antoinette or emergency care, especially when Asthma is not under control. Besides, a pulmonologist will also be helpful for the team to assess for any additional respiratory condition besides Asthma. In addition, an immunologist will also be significant, as they will help identify the allergens that trigger Antoinette's symptoms. The team should also consist of a psychiatrist and a psychologist to help Antoinette and her mother deal with the emotional stress that may come as a result of coping with Asthma with a chronic condition. Lastly, a pharmacist will also be necessary for the team. They will be responsible for recommending safe medication use and side effects that result from using the asthma drugs.

An interdisciplinary approach will provide comprehensive care for Antoinette because they will be treating her in entirety rather than one or a few aspects of her well-being. Comprehensive care will, in turn, lead to improved outcomes and therefore increase the patient’s satisfaction (Ciemins et al., 2016). Besides, her medical care will be of high quality because of the involvement of specialists who are well conversant in their area of expertise, who can work cohesively as a team to improve the productivity of their services. Lastly, it will save her time, and she will be able to go back to school soon since teamwork leads to a streamlined workflow, which saves time. 

What benefits arise for Antoinette and her mother if an interdisciplinary approach to her care is provided?

There will be reduced costs incurred due to shortened hospital stays due to good health outcomes. Moreover, there will be time-saving for both Antoinette and her mother since with an interdisciplinary approach, there are improved outcomes, and therefore, less time will be used for hospital visits (Ciemins et al., 2016). Lastly, an interdisciplinary approach will also improve the mental well-being of both Antoinette and her mother, which in turn may reduce the occurrence of her condition since emotional stress can be a trigger of asthmatic attacks.

Various factors will have significantly hinder an effective team if not appropriately identified and addressed. These include; decreased communication among providers, which can be overcome by strategizing well internally through the implementation of clear lines of communication, for example, carrying out dialogues between all members involved (Dirks, 2019). In addition, lack of appropriate task delegation may decrease job satisfaction. This can be overcome by effectively delegating duties among providers, which will decrease time wasting for both the patient and the providers. Besides, the lack of a proper strategy in engaging the patient will also hinder effective teamwork. It may be overwhelming to the patient, especially if the providers ask similar questions and recommend the same thing. This can be overcome by effective communication across the team (Dirks, 2019). Lastly, lack of meetings among the team members may delay an effective healthcare plan for the patient. This can be overcome by regularly scheduling meetings, which may engage the patient physically and help in scheduling appointments that don't overlap.

Interdisciplinary working involves different disciplines working together and integrating their knowledge to achieve a goal, such as good health outcomes, such as Antoinette’s case. The patient can be treated in entirety rather than a specific body system. A holistic approach to the patient, which is achieved by working as an interdisciplinary, can reduce the length of stay of the patient in the hospital, even reducing chances of getting hospital acquired infections and minimizing healthcare costs (Ciemins et al., 2016). It encourages continuous communication among health care providers, which cements their relationship and improves respect for everyone's contributions in healthcare decision-making, which improves the quality of care for the patient.
