Write a 100- to 400-word blog post that identifies the team at your clinical setting. Discuss how power is manifested in the team. Is the team working well together? If not, what can you contribute to the improvement of the team? Finally, consider how the clinic team works with other community-based resources and comments.

In patient-centered care, various individuals such as the patient, receptionists, physicians, advanced registered nurse practitioners, specialists, hospital staff, ED providers as well as a physician assistant collaborate to ensure improved care (Schottenfeld et al., 2016). Daily, these individuals work together to deliver quality care. Every individual in the team plays an essential role. For instance, the reception is the first point of contact when the patient visits the health care facility. The receptionist is the gatekeeper, mainly to preserve the schedule of physicians. Sometimes the receptionist can also take other roles as ordered by the RNs, such as checking on whether a patient requires routine lab work before a visit, reminding the patient of particular preventive services requirements like immunization. (Lancaste et al., 2015) The RN possesses a specific set of skills and plays a critical role during patient care. The RNs are involved in roles like taking and monitoring the vital signs and taking blood samples as directed by the physician. Physicians are always prepared to manage the health care and medical requirements of patients. The leading roles of physicians include diagnosis, medication prescription, and planning of treatment.

In collaboration with physicians, specialists such as pharmacists play a crucial role in the management of medications prescribed to the patient by the physician. Other specialists, such as psychologists, offer therapeutic assistance while the nutritionists manage the nutritional needs of the patient (Schottenfeld et al., 2016). All these professionals, from the receptionist to the nutritionists, promote timely access to appropriate care, and connect the patient to the relevant health care resources. In my organization, the team collaborates well, and everyone knows his or her roles. The team can, however, be improved by increasing physician-patient communication to enhance compliance with the treatment regimen for better health care outcomes. The team can also be improved by enhancing the role of every individual and promoting communications among the team members as this will positively enhance patient safety, quality of outcomes, patient satisfaction as well as job satisfaction of the health care professionals and specialists (Lancaste et al., 2015). Besides, the team needs to collaborate with the community–based care coordinators to know the population of patients and ensure that patients receive the needed care services to maintain a stable, healthy population and minimize further expenditure on care services.
